The 2021 SALA Student Request form (undergraduate and honours) says to complete all fields, attach your academic record, and hand in the original to SALA: Registration and Student Record Management. The faculty gives a recommendation and the executive dean makes the final decision. It is your responsibility to copy the form before handing it in and to find out the decision yourself. For a termination reconsideration, Rule 1.17 says to use the prescribed form via the student portal, so ask your faculty office which applies.
