If illness affected your results, UP says to include medical certificates or doctors' letters. If the faculty readmits you on that basis, it may expect you to manage the condition responsibly. If mental health was a factor, you can approach the Student Counselling Unit, which can offer one online session and point you to community services. It cannot arrange readmission.
