Include a written motivation that explains what affected your performance and what you will do differently if readmitted, plus supporting documents such as medical certificates, affidavits or other evidence. Be complete the first time: if you leave out material information you cannot add it later if the appeal fails, and incorrect or fraudulent information lets the committee reconsider its decision.
